Resources to Improve Referrals from Part C

Early Identification and Referral

If you are interested in collaborating with Part C to boost the number of children referred to your project, here are 2 resources that can be found in the EI/R Tool Kit that are a "must" to check out:

1) Risk List for Combined Vision and Hearing Loss in Infants and Toddlers

This is a four-page document that lists etiologies of deaf-blindness with their associated ICD-10-CM (International Classification of Diseases, Tenth Revision, Clinical Modification) codes. (ICD-10 replaced ICD-9 in October 2015.) The target audience is Part C service coordinators. The document includes space to insert state specific project information.

2) Early Identification and Referral PowerPoint

This is a PowerPoint developed by the Missouri Deaf-Blind Project for Part C Single Point of Entry Directors. Delivered as part of annual presentation for APH (American Printing House for the Blind) quota registration.

Looking forward to hearing back about whether you find these helpful in your work within your state! I know that they made a big difference in boosting referrals in MO!
