Age 22 to 26

The need for transition preparation does not end at age 21. Best practices during the age 22 to 26 age range focus on advancing and sustaining achievements from earlier transition preparation phases, including:

  • Ensuring appropriate assessments have been completed and documented
  • Continuing to advocate for accommodations and supports that meet the individual’s interests and needs
  • A focus on employment and community connections

To the greatest extent possible, the team should continue networking and involve the young adult (e.g., via e-mail, social media).
